Wellness Program forms due in November

Faculty and staff who are currently using the university’s medical benefits and who completed the three onboarding activities in fall 2019 (biometric screening, health risk assessment and tobacco attestation) are reminded that up to $200 in extra wellness incentives may be available this December.

Wellness participants who completed programming in any of the six Wellness Program categories during 2020 should attest by Friday, Nov. 30.

Throughout the year, programming was offered both online and in-person in community, financial, nutrition or weight management, physical activity, stress management and tobacco cessation. In most cases, successful program completion means attending or participating in 85% of offered classes in a series. For classes offered via Zoom, watching the recording sent after the class can also be counted for participation. All participants in Zoom classes should be sure to fully update their online attendance sheet in the event of an audit. Wellness Program participants—those who are receiving the $25 monthly incentive this year—can earn an additional $100 for one program or $200 for two programs by completing programming and attesting. While participants are welcome to complete programming in as many areas as they like, the maximum annual payout is $200 in the final December paycheck.
