Wellness attestation forms now available in HCM

Faculty and staff Wellness Participants who have completed classes in stress management, physical activity, nutrition or tobacco cessation at some point this year may now attest to program completion for 2016. Participants can receive $100 for one program or $200 for two program completions in 2016. Attestation forms for the Wellness Program Incentives will remain open and available for completion through November 30.

Criteria details are available on the wellness website. Common 2016 options include:

  • Successfully completing HealthTrails (600 virtual miles traveled) qualifies for the physical activity incentive.
  • Attending seven of eight classes in the S.M.A.R.T. 1, 2 or 3 series; mindfulness series; or meditation series qualifies for the stress management incentive.
  • Attending seven of eight classes in S.M.A.R.T. Eating or Get Fit Together with Nutrition qualifies for the nutrition and weight management inventive.
  • Completing tobacco attestation either through the QuitLine or the Selfhelpworks LivingFree class qualifies for the tobacco attestation incentive.

To attest today, log onto HCM and click on Main Menu, Self Service, Benefits, Wellness Program Participation. Then using the tabs on the top of the page, attest in the area in which the program was completed.
