Sign up now for biometric screenings as first step to qualify for 2016 wellness incentive

Faculty and staff members now can register for biometric screenings, which will be offered on campus beginning Sept. 15. The screening will help participants qualify for the 2016 wellness incentive.

Employees are encouraged to sign up for the first few weeks of the screenings, which are more open and accessible.

Register at we.blueprintforwellness.com and use the registration key CWRU to get started.

Follow these steps to register:

  1. Log in to your account—or create a new account.
  2. On the welcome screen, click on “Your Screening.”
  3. On the “Your Screening Page,” click on “Schedule Now.”
  4. Click on the “At An Event” button.
  5. Select your preferred building—Tinkham Veale University Center, Crawford Hall or Frances Payne Bolton School of Nursing.
  6. Select the day and time.
  7. Put it on your schedule.

The Health Risk Appraisal and Tobacco Attestation Form are also required to qualify for medical plan participants to receive the monthly Wellness Premium Incentive in 2016. The forms will be available beginning Sept. 1.
