Participate in wellness activities to improve health and qualify for monthly premium incentive

Learn more about your health by completing three wellness activities this fall: a biometric screening, Health Rick Appraisal and Tobacco Attestation Form. All benefits-eligible faculty and staff can complete the screening and Health Risk Appraisal, however only employees who use one of the university’s medical insurance plans will be able to receive the $25 monthly Wellness Premium Incentive in 2016. To receive the benefit, eligible employees must complete three activities by Nov. 30.

Biometric Screening

Registration is now available for the biometric screening at Use registration key “CWRU” and sign up for one of the on-campus screenings offered Sept. 15 through Nov. 30 in three campus locations: Tinkham Veale University Center, Crawford Hall and the Frances Payne Bolton School of Nursing. Registration closes two weeks prior to each screening day. Sign up now to make sure that you get this important step completed.

Completing the biometric screening will give you helpful information about your health. The free screenings include blood pressure, height, weight, waist circumference, cholesterol and glucose tests. Fasting for nine to 12 hours in advance is necessary.

If none of the on-campus screening dates are convenient for you, visit the registration site to find a Patient Service Center location in the Cleveland area.

If you have had all of the above screening tests completed since April 1, you may use this form (PDF) to share your results with Quest. The form must be signed by your health care provider and faxed to Quest prior to Nov. 30 to count towards this participation requirement.

Individual results will remain confidential. The university will receive only de-identified, aggregate results. Additional details about the screening and registration process can be found within the wellness website.

Health Risk Appraisal (HRA)

The University of Michigan administers this comprehensive, private health questionnaire online. A personalized health risk profile is created following completion of the questionnaire and the biometric screening process. A secure process is used to include biometric screening data within the HRA. Faculty and staff will receive a confidential results report. The university cannot access individualized data; it only receives a de-identified aggregate report.
