Earn extra money next year with the CWRU Wellness Program

The Case Western Reserve University Wellness Program encourages all benefits-eligible faculty and staff to participate in the annual Fall Wellness Activities: Biometric Screening, Health Risk Appraisal and Tobacco Attestation.

Those who complete all three activities, and who select one of CWRU’s medical insurance plans, will receive $25 additional pay each month in 2017. In addition, they become eligible for an extra $100-200 by participating in another program during the year.

Measurements taken during the Biometric Screening include: blood pressure, cholesterol, fasting glucose, height, weight and more. Screenings are conducted on campus from the end of September through Nov. 30. Participants are asked to pre-register at my.questforhealth.com/ by using the registration key “CWRU.”

Those who have recently had these tests completed by their primary care provider can select the Physician Result Form from the Quest website.

The Health Risk Appraisal or HRA is a comprehensive and private questionnaire that creates a personalized risk profile. The HRA is available online and can be completed anywhere with secure internet access. At just over 60 questions, this test takes about 10-20 minutes to complete.

The Tobacco Attestation form in HCM simply verifies one’s tobacco use status. Those who do not use tobacco will indicate so. Those who do will indicate that they do use tobacco. In order to receive the monthly Wellness Premium incentive, tobacco users must completed a cessation class in 2017. To find the form, go to hcm.case.edu and navigate: Main Menu – Self Service – Benefits – Tobacco Attestation Form. Be sure to indicate attestation for the year 2017.
