Complete 2025 Wellness Incentive Program activities by Nov. 30 deadline

The deadline is approaching for faculty and staff who would like to earn a $25 Monthly Wellness Incentive in their pay in 2025. Those interested must opt into one of the university’s Benelect medical plans for next year and complete three eligible activities this fall. The activities outlined below must be completed by Nov. 30.

To earn the incentive, employees must complete the Health Risk Assessment on the WebMD site, plus choose two of these three options:

  1. Biometric Screening: Complete at an on-campus screening event (appointments are still available), a Quest Diagnostics Patient Service Center, or through your primary care provider with results submitted via Quest’s Physician Results Form. Login to the Quest site with CWRU Single Sign On to schedule your appointment or download a Physician Results Form. The Quest screenings are free and available to all benefits-eligible employees, even if you are not seeking to earn incentives.
  2. Tobacco Attestation Form: Available in HCM in the “Wellness” tile.
  3. Primary Care Provider Attestation: Available in HCM in the “Wellness” tile.

The Wellness Incentive Program is voluntary. Earnings are taxable and only available to those who opt into a university medical plan for 2025.

Those who qualify for the $25 Monthly Wellness Incentive this fall will have the opportunity to earn an additional $100-200 in Wellness Program Incentives in 2025.
