Biometric screenings (blood pressure, height, weight, cholesterol, glucose – via venipuncture) can still be scheduled online at my.blueprintforwellness.com (use key code CWRU) for visits to a Quest Patient Service Center on or before Dec. 6.
Scheduling for an onsite screening is now closed. The deadline to complete the University of Michigan Health Risk Appraisal and the Tobacco Attestation Form also has passed.
Employees can track their participation status for each of the three wellness-engagement opportunities by logging on to the HCM system at case.edu/hcm. Click on Main Menu – Self Service – Personal Information – Personal Information Summary.
At the bottom, each of the three activities is listed. Employee participation is signified when the box and date fields next to each item have been filled in.
There may be a weeklong delay for the Health Risk Assessment and screening participation to be documented in this area of HCM. Upon completion, the Tobacco Attestation Form should immediately show within HCM as completed.
