Wellness attestations due by Monday, Nov. 30

Faculty and staff are reminded that the three onboarding activities to earn the monthly Wellness Incentive in 2021 are due by Monday, Nov. 30. The activities include a Health Risk Assessment, and two of the following three activities:

  • Biometric Screening (at a Quest Patient Service Center or with the Physician Results Form)
  • Tobacco Attestation
  • Primary Care Provider Attestation

Enrollment in one of the university’s medical plans is also necessary to receive the incentives. In addition, those who completed 85% of the classes in any of the 2020 Wellness Program series should attest to Wellness Program Completion under the Wellness Tile in HCM. Programs took place between Jan. 1 and Nov. 30 in all program areas:

  • Community Well-being;
  • Financial Well-being;
  • Nutrition and Weight Management;
  • Physical Activity;
  • Stress Management; and
  • Tobacco Cessation.

Those who attest and are already receiving the monthly wellness incentive will see an additional $100-200 in their final 2020 paycheck. See the wellness website for more details about the Wellness Program incentive.

The Nov. 30 date is the cutoff with no exceptions, so don’t miss the chance to learn about and improve your health and earn a little extra money next year.
