Receive benefits info at final presentation; important wellness program deadlines approaching

The final presentation on the new wellness program for employees taking part in the Benelect medical plan will be held Thursday, Oct. 31, from noon to 1 p.m. in Allen Memorial Library’s Ford Auditorium.

Benefits open enrollment updates will be shared, in addition to details on the university’s new wellness program. Those interested should attend to learn more about participation opportunities available now and in November for faculty and staff with Benelect medical plan coverage, or those considering adding coverage for 2014.

Also, Friday, Nov. 1, is the first day medical plan participants can complete the Health Risk Assessment and Tobacco Attestation form online. All Benelect medical plan participants, and those considering adding the insurance for 2014, will be able to complete the Health Risk Assessment and Tobacco Attestation Form online Nov. 1-30 using the links and information below.

There is one more week left to schedule an onsite biometric screening. The link below allows employees to schedule an onsite screening or a visit to a Quest Patient Service Center.

Those who have had a screening done since April 2013 and want the results sent to Quest should email wellness@case.edu.

Completing all three steps above by Nov. 30 will allow medical plan participants to receive a $25 per month premium incentive in 2014. Questions should be directed to wellness@case.edu.
