PRCHN Seminar Series: “The Better Health Pathways Community HUB”

The Prevention Research Center for Healthy Neighborhoods (PRCHN) will continue its seminar series with a presentation on the “Better Health Pathways Community HUB” Wednesday, Oct. 14, at noon via Zoom. Rita Horwitz, president and CEO of Better Health Partnership, will present the seminar.

In 2020, Better Health Partnership launched the new Pathways Community HUB (PCH) for Cuyahoga County—becoming one of 10 Pathways HUBs in the state of Ohio, supported by the Ohio Commission on Minority Health. The PCH is a centralized, evidence-based, care coordination model that improves health, reduces costs and promotes equity.

This presentation will describe the Pathways HUB model and the value it brings to Cuyahoga County’s most vulnerable residents. Horwitz will explain how strategic partnerships with health systems and community organizations can optimize outcomes for targeted populations with challenging health and social needs.

The PRCHN Seminars aim to foster conversations and new collaborations, create opportunities to address health concerns, and aid in the work to achieve health equity in our communities. The PRCHN invites all community members and organizations and CWRU staff, faculty and students to join in watching the seminars.
