Employees: Begin submitting fall tuition waiver applications now; deadline Sept. 30

The deadline for submitting fall tuition waiver applications to the Office of Human Resources is Sept. 30, but employees are encouraged to send in their materials as soon as possible.

Reminder: Employees using tuition waiver for a dependent child must include the annual proof of dependency with your paperwork.  Accepted forms of proof are as follows.

  • For a child who is your tax dependent: a copy of the portion of your federal income tax return where the child is listed as a dependent, or
  • For a child who is not claimed on your taxes: a completed Child’s Proof of Dependency Affidavit

Links to all necessary forms can be found under the “Education” heading on the benefits forms webpage. Employees may scan and email forms to tuition-waiver@case.edu, fax them to 216.368.3582, or bring them to Benefits Administration in room 224 of Crawford Hall. Please do not include social security numbers on any forms sent via email.

Tuition waivers are always posted to students’ SIS accounts after the end of the drop/add period. For fall 2012 the process will begin Sept. 10 and continue throughout September and early October.

Please visit the education benefits website for complete information, including links to the tuition waiver policy/procedure, related forms, and a benefit summary.
