Deadline to complete Wellness Program requirements Nov. 30; check HCM for record of participation

All three Wellness Program activities need to be completed by faculty and staff members by Sunday, Nov. 30, to qualify for the $25 monthly Wellness Program incentive for 2015.

The three requirements that need to be completed are:

  • Tobacco Attestation Form in HCM: (HCM – Main Menu – Self Service – Benefits – Wellness Program)
  • Health Risk Appraisal at case.edu/hra
  • Biometric screening

On-campus health screenings will be held Nov. 17-20 in the Tinkham Veale University Center’s Senior Conference Room from 7 a.m. to 2 p.m. each day, and Nov. 24-26 in the School of Nursing’s Faculty/Staff Lounge from 7 a.m. to 2 p.m. each day.

The deadline for scheduling an on-campus screening has passed, but those individuals planning a walk-in screening should register online at We.BlueprintForWellness.com with the registration key CWRU. Though it is no longer possible to schedule a screening for an on-campus date, online registration will provide online access to screening results within three to five days of screening and will confirm a home address so a hard copy of the results can be sent to participants’ homes within two to three weeks of the screening date. Note that for screenings, fasting is required for nine to 12 hours prior, with the exception of water and black coffee.

There are only seven days left to get a screening done on campus as a walk in, 13 days left to get a screening done at a Quest Patient Service Center and 13 days to get a Physician Results Form sent to Quest with data gathered by a primary care provider since April 1.

Faculty and staff completing the wellness activities can confirm that their participation has been recorded in HCM by logging on and going to: Main Menu – Self Service – Personal Information – Personal Information – Summary and scrolling down to see the three wellness activities listed.

When there is a check mark and date next to each item, participants will know the benefits office has received confirmation of their participation.
