The Office of Human Resources has added more dates and times for on-site biometric screenings, which are part of the new employee wellness incentive program. Additional on-site screenings will take place Dec. 2-6, from 7 to 11 a.m. in Crawford Hall, Room 209. All screening participants must fast for 9 to 12 hours before the screening.
Registration for each screening must be completed two weeks prior to the screening date. The deadline to schedule an on-site screening (for the Dec. 6 date) will be Nov. 22. Faculty and staff members also can visit a Quest Patient Service Center through Dec. 6. Both options can be scheduled at my.blueprintforwellness.com using key code “CWRU”.
Although the screenings may take place after the Nov. 30 due date, the Tobacco Attestation Form and the Health Risk Assessment still must be completed by Nov. 30. Employees obtaining a screening in the first week of December will have their screening results automatically loaded into their health risk assessment form, so there is no need to wait to complete that requirement.
Also, employees can track their completion status for each of the three wellness-engagement opportunities by logging on to the HCM system at case.edu/hcm. Click on Main Menu – Self Service – Personal Information – Personal Information Summary. At the bottom, each of the three activities is listed. Employee participation is signified by a filled in circle and a date added next to each item.
There may be a week-long delay for the Health Risk Assessment and screening participation to be documented. Upon completion, the Tobacco Attestation Form should immediately show within HCM as completed.
