Medicine’s Colin Crowe publishes article in Cardi-OH publication

Colin Crowe, assistant professor at the School of Medicine, published an article titled “Connecting Patients with Diabetes to Pathways Community HUBs to Address Social Needs,” in a December issue of Capsules, a publication from the Ohio Cardiovascular & Diabetes Health Collaborative (Cardi-OH). 

Unmet social needs represent a major barrier to successful diabetes management and control. The article discusses how a referral to the Pathways Community HUB model can help address a patient’s social needs.

Contributing authors include Joshua J. Joseph of The Ohio State University and Susan S. Davis of University of Cincinnati.

Case Western Reserve University is the lead institution for Cardi-OH, a statewide initiative of healthcare professionals at Ohio’s seven medical schools who share knowledge to improve Medicaid patient outcomes and eliminate health disparities across Ohio.
