Recent doctor visit may meet biometric screening requirement

Each fall, benefits-eligible faculty and staff are encouraged to take part in onsite screenings through the Wellness Program. These screenings offer a snapshot of health, and participants are encouraged to share results with their physician. This can potentially alleviate the need to repeat tests.

For those who have had their annual doctor visit any time after April 1 of this year, a Physician Results Form may be used in place of the onsite screening.

To print a personalized Physician Results Form or to register for an onsite screening, log onto the Quest website. New registrants should use Registration Key CWRU.

Once logged in:

  • Click on “Get Started”
  • Then “Participate Now”

There will be three options from which to select:

  • At an Event: Select this option to participate on campus.
  • At a Patient Service Center: Select this option to have it completed off site.
  • Physician Results Form: Step-by-step instructions (PDF) for using the Physician Results Form available online.

Benefits-eligible faculty and staff who will be participating in Benelect coverage in 2017 are eligible for a $25 monthly wellness incentive by completing the biometric screening, the Health Risk Appraisal and a Tobacco Attestation each fall. Details can be found on the Wellness website.
