Earn an extra $300 in 2018 with Wellness Premium Incentive

Faculty and staff who carry university health insurance have the opportunity to earn $25 a month in 2018 with the Wellness Premium Incentive by completing two simple steps this fall: the biometric health screening and tobacco attestation.

Biometric health screenings

Screenings take place on campus through Nov. 30. Participants should register online for an on-site screening in the Tinkham Veale University Center, Crawford Hall or Frances Payne Bolton School of Nursing. Registration details are available on the Wellness website.

Time is running short for those who prefer the Tinkham Veale University Center location—scheduling is only available for Nov. 9 and 10.

Tobacco attestation form

Completing the Tobacco Attestation Form can be done from any computer.

To fill out the form, participants can:

  • Log into Human Capital Management (HCM);
  • Navigate from Self Service to Benefits to the Tobacco Attestation Form; and
  • Click “save” and “OK.”

Even those who do use tobacco can be eligible for the wellness incentives. Those who attest to being a tobacco user will need to complete a tobacco cessation class before Nov. 30, 2018. Upon completion, participants will contact Elizabeth Click, medical director, at erc10@case.edu so that she can verify program completion. Then, all the incentive dollars for 2018 will be added to the paycheck. Studies show that it can take many attempts to successfully quit, so the Wellness Program supports any effort at cessation.
