Complete Tobacco Attestation Form, biometric screening by Nov. 30 to be eligible for Wellness Premium Incentive

This fall, it will be a little easier for faculty and staff to earn their $25 a month Wellness Premium Incentive. Because the annual Health Risk Appraisal is on hold, participants will only need to complete the Tobacco Attestation Form and Biometric Screening before Nov. 30 in order to be eligible for the incentive.

The Tobacco Attestation Form is available now at case.edu/hcm. The form can be found by navigating from: Main Menu – Self-Service – Benefits – Tobacco Attestation Form. Participants should attest for 2018. In some cases, this will require scrolling to the bottom of the attestation form and clicking on the “Add Next Benefit Year Button.” Then, attest to your current tobacco use status and click “Save.”

The Quest scheduling system for the biometric screening also is open. Times, dates and registration details are on the Wellness website at case.edu/wellness/facultystaff/the-wellness-program/wellness-premium-incentive/biometric-screening/.

The Health Risk Appraisal, which incentive recipients completed the last four years, is on hold for the fall of 2017 and therefore not a requirement for Wellness Premium Incentive participation for 2018.

All faculty and staff who participate in one of the university’s medical plans are eligible to earn an extra $300 next year by simply completing these two easy steps by Nov. 30.
